Hospice of the Conejo does not receive any funding from Medicare or Private Insurance; all of our funding is raised through private donations, grants, and fundraisers. We truly appreciate and need your dedicated financial support to continue to provide free-of-charge compassionate care for hundreds of individuals facing life-limiting illness and bereaved families in the community. We have been providing our services free of charge since 1977.

Tree Of Life Star Dedication

The Hospice of the Conejo Tree of Life stands as a symbol of love and remembrance during the holiday season. Dedicate a Star Ornament to honor or remember your loved one. Names of loved ones are written on the Stars and hang on the Tree of Life during the holiday season (Thanksgiving until Christmas Eve). Donations are used to serve those in our community who are terminally ill, as well as their families, and provide comfort and healing support to those who have lost a loved one.

A Star Dedication Card for the Tree of Life can be sent with a minimum of $20 donation. We appreciate your generous tribute for any amount over this minimum. The amount will not be disclosed to the recipient. Please make sure you complete the PayPal form on the next page to ensure the Dedication Card will be sent.

Workplace Giving

If your employer engages in a workplace giving campaign, you may be able to designate payroll-deducted donations to Hospice of the Conejo. We are a qualified 501(c)3 organization that accepts tax-deductible donations.



A Memorial gift is a way to pay tribute to an individual's life. Your friend, colleague, or loved one's family will be notified of your thoughtfulness through a personalized acknowledgement card. You may make a memorial gift using this form or make your memorial gift online.



Join our monthly giving program and have your donation spread out over time. You can securely set up your pledge online and have your gift of $20, $50, or $100 automatically donated to HOC each month.
